Model Commitment Acknowledgments

Acknowledge each required statement.

I will complete required intake, candidacy assessment, authorization, and procedure-specific informed consent. I will attend the consultation, treatment, and every required follow-up and photography appointment. I understand model appointments may require additional time for planning, documentation, photography, or video. I will follow preparation and aftercare instructions and promptly report unexpected symptoms. I understand selection, treatment, outcome, symmetry, duration, or complete correction is not guaranteed. I understand healing, refinement, additional product, or additional treatment may be required at additional cost unless included in writing. If selected, I understand a $50 reservation deposit will be collected and applied in full to treatment. If the clinical assessment determines that treatment is not appropriate, the deposit will be promptly refunded. I will arrive on time and comply with applicable cancellation and rescheduling requirements.

Required Photography & Public Marketing Authorization

Public image use is a required condition of model participation. The purpose of preferred model pricing is to build LUVION’s clinical portfolio and create authentic educational and marketing content.

This authorization includes identifiable treatment-area images and, when captured, full-face photographs and video.

No compensation or royalty is owed beyond the written model-pricing terms.

Images may be edited for cropping, lighting, and layout but not to materially misrepresent results.

Authorization may be revoked prospectively in writing; content previously lawfully published, distributed, or used in marketing may not be retractable.

LUVION will not publish a name or personal health details unless separately authorized.
